Aetna ARRA Attestation Form
For new Aetna groups who have members currently enrolled on state continuation, Aetna will require a copy of the attestation form so they can properly continue to administer the 35-percent premium
payment for the AEI. They will need this form to receive the appropriate payroll tax credit and will accept a copy of the previous
carrier’s attestation form in place of the Aetna Attestation form.
